Why: One of Oak Middle School’s goals is to encourage students to make good decisions, to grow as a team and for students to work cooperatively to solve problems. Attending Teamworks will help build team identity, strengthen communication and decision-making skills as well as provide an opportunity to reflect on the core values of Oak Middle School.
Details:
- We will travel to and from Teamworks by school bus.
- We will participate in high-energy physical activities, so appropriate dress of sneakers and lose fitting clothing is required.
- Students are responsible for bringing a bagged-lunch. No glass please!
- Cost: $22.00 -Please make checks payable to “Town of Shrewsbury” and put "OMS Student Activity" and your child's name on the memo line.
- If the cost is an issue with your family, please contact Mrs. Burak as she may be able to direct you to financial assistance, as we would like all students to be able to participate in the day.
- If you would like to contribute to a scholarship, please include any additional funds with your payment.

- I give permission for my son/daughter to attend Teamworks. I understand that he/she will be participating in physical activities and the school is not responsible for any injuries that occur on site.
- I do NOT give my son/daughter permission to attend Teamworks.
I understand that if my child is prescribed an epi-pen, he/she is required to bring it to the Health Office prior to his/her trip. The health office MUST have an Allergy Action Plan on file for any student needing an Epipen.
