HOW TO REGISTER FOR FIELD HOCKEY TRYOUTS

To register, there are several steps required. Please complete all steps below:

• Parents will need to go to the Parent Portal on PowerSchool and click on the Sports, Activities, Permission Slips tab on the left. Choose Oak Middle School Sports  and click on the link for the “Colonial ID Form” at the top of the page and complete all the required information. To submit, click the blue button that states "I certify that this form is being submitted by both parent/guardian and the student-athlete named above".

• Return to Middle School Sports page and check the Field Hockey Tryouts from the list and click SUBMIT. The $100.00, per sport, athletic fee DOES NOT have to be assigned or paid in order to tryout, but must be paid if your child is selected.

• The Concussion Training Manual referred to on the Colonial ID Card can be found at the HERE. Both parents and students will need to read and sign off that they have read the Concussion Manual (sign off is located on the bottom of the Colonial ID form).

• Provide a copy of a physical exam completed on or after August 1, 2020. This paperwork should be dropped off to the School Nurses’ office.

• All forms/fees need to be completed in full by September 6th to participate in the program.
