SPS Vaccine Clinics This Week for SPS Students & Staff & Their Families

This message is for all Shrewsbury Public Schools Families and Staff:

Our school district is hosting free COVID-19 vaccine clinics on Monday, 12/20, Tuesday 12/21, and Wednesday 12/22, from 4pm to 7pm at Oak Middle School in collaboration with the Central Massachusetts Regional Public Health Alliance. Students and others who received their first vaccine dose at our clinics earlier this month will be able to receive their second dose.  Please also note:

  1. Due to an issue with the online registration portal that was used for the first round of clinics, several individuals who need their second dose were not able to make appointments.  As a result, those who need their second dose and who have not been able to register for an appointment may attend any of the three clinics as a “walk up” without an appointment.  If you already have a confirmed appointment for your child or yourself for one of the three clinics, please keep that appointment date and time. 
  2. We anticipate there will be ample supply of vaccine for a) doses for ages 5-11, b) initial doses for ages 12 and up, and c) booster doses for those who are eligible and have already had two shots; therefore, any SPS students and their family members and SPS staff members and their family members who would like to receive a vaccine may attend any of the three clinics on a walk-up basis.
  3. Because of walk-ups and anticipated high demand, please expect longer wait times.

Initial series Pfizer vaccines are available for children ages 5 - 18 and Pfizer boosters are available for children ages 16 -17.  Pfizer, Moderna and J&J boosters are available for all adults (ages 18 and up) who have received their initial series.  At this time, we do not have additional vaccine clinics scheduled at school.

A link to download the pre-screen registration form is below. If you or your child will be attending but do not already have an appointment, please print, complete and bring the form with you to the walk-up clinic to allow for a faster check-in process.  If you were able to make an appointment online for 12/20, 12/21, or 12/22 and received a confirmation, you do not need to complete the linked form.  

Please enter Oak Middle School at the front of the building, using the doors on the left.  Proceed straight forward to the cafeteria.

Name of Insurance Company & other insurance information if available.  Insurance is not necessary to receive the vaccine.

Please sign the bottom next to the dose you are receiving #1, #2 or #3 and fill in date of clinic you are attending (12/20, 12/21 or 12/22). Parents, when you sign this form you are giving permission for vaccine administration to your child.
