This message is for all Shrewsbury Public Schools Families and Staff:
Our school district is hosting a free COVID-19 vaccine clinic on Thursday 1/13/22 from 4pm to 7pm at Oak Middle School in collaboration with the Central Massachusetts Regional Public Health Alliance. Students and others who received their first vaccine dose at our clinics last month will be able to receive their second dose. Please also note:
- Due to an issue with the online registration portal that was used for previous clinics, pre-registration is not available. This clinic will be a “walk up”clinic.
- We anticipate there will be ample supply of vaccine for a) doses for ages 5-11, b) initial doses for ages 12 and up, and c) booster doses for those who are eligible and have already had two shots; therefore, any SPS students and their family members and SPS staff members and their family members who would like to receive a vaccine may attend this clinic on a walk-up basis.
- Because of walk-ups and anticipated high demand, please expect potentially long wait times.
- Please bring your vaccine card with you if receiving a second dose or booster dose.
Initial series Pfizer vaccines are available for children ages 5 - 18 and Pfizer boosters are available for children ages 12 -17. Pfizer, Moderna and J&J boosters are available for all adults (ages 18 and up) who have received their initial series.
A link to download the pre-screen registration form and instructions for completing the form are below. If you or your child will be attending, please print, complete and bring the form with you to the walk-up clinic to allow for a faster check-in process.
Please enter Oak Middle School at the front of the building, using the doors on the left. Proceed straight forward to the cafeteria.
Information regarding additional clinics being offered by the Central Massachusetts Regional Public Health Alliance can be found here. Clinics are currently scheduled in Worcester locations on 1/10, 1/12, 1/14 and 1/15.
