Please note, beginning with the 2015-2016 school year, this agreement only applies to those who were enrolled in the Personal Technology Fee option in grades 5-7 during the 2014-2015 school year and continue to opt for that option going into the 2017-2018 school year in grade 8.

As of the 2018-2019 school year, this option is no longer available.

To keep the devices in good working order and sustain the program we need a clear and equitable way to share liability that protects the families and the program. Our goal is to minimize repair and replacement costs by ensuring that we all have a financial stake in treating the iPads responsibly, with care and respect, and without unduly penalizing honest misfortune.

Please read and agree to all the obligations and expectations set forth for the Personal Learning Device Program option that you have chosen for your student as outlined below.

1. I will be responsible for maintaining the device and all of its accessories--case, charger, and sync-cable--in good working order through the life of the program and will be responsible for purchasing replacements through the school for any accessories or parts that get lost or broken.

2. I understand that I will be responsible for reimbursing the schools for repairs or replacement as follows:

  At Home

At School during school hours

Damage done on purpose

Full cost of repair

Damage due to carelessness or negligence (includes removing device from district case and/or traveling without device in protective cover or sleeve)

3. I understand that if I unenroll my child from the Shrewsbury Public Schools for any reason, I am obligated to immediately return the device and all of its accessories.

4. I understand that the annual technology fee is a fee-for-use and that the district makes a good-faith financial commitment based on my registration and as such the fee is not refundable or pro-ratable for any reason.

5. I understand that I may be assessed a $25 late fee for payments not received by the published deadline.
