SUMMER PROGRAM FORMS

ALL students must submit an emergency/health history form in order to participate in summer enrolllment.  You can download the form from the "forms" submenu.

If your child will require medication while at the enrichment program, please download and complete the medication order form.  You will have the opportunity to drop off medication with the nurse prior to the start of the program.  There will be a date in late June, to be determined, where the nurse will be available for medication drop off and consultation.
